Obviously over time we'll be able to control when the transformations occur, but what about in the initial stages?
The initial transformation is going to be the only one that fully takes place outside of the characters control.

After that, you'll be okay for a few days, but then the monster will start to surface again. The mostly likely expression of the monster is likely to be hungry, such as Isaac and Griz. However, if you wish it to manifest in a different way, you can.

Like any desire, your character will be able to suppress it. However, early in the game, it will be very telling. After awhile (a few days to a week), the urge will be almost overwhelming, like a junkie craving a fix. It will be hard to contain it, but possible.

It just an indicator that something is up. At first, you won't even know what the heck it is supposed to mean. Call it a range of about 100 yards to sense...get within 20 feet and you can pinpoint it.
