And yeah, a lot of people theorize that the thorns of Oglogoth or causing Rose to lose Sanity. In fact, Rose is the Seer of Light. She is seriosuly mixing things up.

It's probable that the second half of their titles are less a matter of "Element" or "Nature" and more a simple matter of where their first gate was.

John - Breath - Land of Wind and Shade
Dave - Time - Land of Heat and Clockwork
Rose - Light - Land of Light and Rain
Karkat - Blood - Land of Pulse and Haze

Edit: Also possible that their powers are based off the land. Dave's got all the clock gears so he gets time travel. Rose's Land could aptly be described as "magical" in every sense of the word.

We don't know what, if anything, John can do outside of use his Hammerkind (and bunnykind) specibus, but his Land seems rather "normal", and thus may himself be. On top of that, it's the only Land we've seen with anything resembling civilization which may lend credence to John's probable, eventual role as the Leader.

Well, as far as civilization goes, that one goddamn stupid civilization. In other words, I don't think he's leader by virtue that his planet is home to tribes of dumb salamanders. Hell, the inhabitants of Rose's planet are probably smarter, and they've been dead for millions of years.
Yeah, I'm guessing the fact that he's leader has something to do with his character, or power, or the fact that he entered the medium first.

And yeah, the titles seem to correspond to the planets.
But does a player's planet define their title define their power?
Or is it the other way around, is the power inherent, and that in turns defines their planet and title? Cause I think it's the latter.
